Case Management at Care Resource is a client-centered collaborative process that works with patients to meet their health and care goals. Through assessing, planning, implementing, coordinating, monitoring and evaluating available options and services, case managers help assure one receives excellent care from available resources. Anyone may ask for Case Management at Care Resource.
Additionally, case managers encourage self-care and adherence to one’s medication regimen. Through support and emphasis on client directed goal setting, case managers empower Care Resource patients to remain as independent as possible. Moreover, Case managers refer people to many community services including primary medical care, prescription drug assistance programs, housing assistance, food programs and many others.
The Ryan White HIV/AIDS Program works with cities, states, and local community-based organizations to provide services to an estimated 536,000 people each year who do not have sufficient health care coverage or financial resources to cope with the HIV condition.
Care Resource is a Federally Qualified Health Center (FQHC) that provides comprehensive primary medical and preventive care, including oral and mental health/substance abuse services to all individuals in our community.
