Case Management

Case Managers help their clients unravel the sometimes-confusing web of access to community services. Services may include primary medical care, prescription drugs, housing assistance, food and many others.

Case Management is a client-centered collaborative process that works to meet clients’ health and care needs. Through assessing, planning, implementing, coordinating, monitoring and evaluating available options and services case managers help assure continuity of care.

Additionally, case managers encourage clients’ self-care and adherence to their medication regimen. Through support and emphasis on client directed goal setting, case managers empower clients to remain as independent as possible, and most importantly, improve the quality of their lives.
