DESCRIPTION:
Responsible for the delivery of appropriate psychosocial and substance abuse therapy services to Care Resource clients as determined by supervisor. Some of the essential duties include: Obtains informed consent prior to providing services. Conducts client Orientation to Treatment Program according to applicable laws and agency procedures. Provides psychosocial and/or substance abuse treatment, including assessment and diagnosis, crisis intervention, treatment planning and reviews for both individuals and groups. Serves and maintains an active caseload as assigned by the supervisor with concomitant productivity of at least 22 billable hours weekly. Adheres to agency procedures and protocols in provision of effective delivery of Mental Health and Substance Abuse Counseling and maintenance of standards of care. Maintains updated and complete treatment plans, progress notes, monthly progress reviews and other required information in client records as specified in performance standards. Determines client need for specialized services within or outside of the agency. Coordinates with client’s case manager for client support services as needed. Facilitates drug urinalysis for court mandated clients following agency procedures. Coordinates court referred programs by writing reports and progress notes as required. Inputs client information using specific software as required. Provides clients with information about bill-coverage, services and procedures as required.
QUALIFICATIONS:
Master’s Degree in Social Work or related field required. Between three to five years of clinical experience are required. Experience in mental health and substance abuse service provider organizations is highly desired. LCSW in the State of Florida required. Medicaid and Medicare providership preferred. Bilingual (English-Spanish/ English-Creole) is highly recommended. Computer knowledge should include Microsoft applications (Word, Excel and Outlook) and Casewatch experience helpful. Excellent communication, organizational and teamwork skills are required to establish and maintain effective relationships with staff, external entities and clients. Excellent decision making and problem solving skills are required. Ability to work with multicultural and diverse population is required.
SALARY: $35,000- 48,000

| TESTING AND TREATMENT ACCESS COUNSELOR (PER DIEM) |
DESCRIPTION:
Responsible for providing Testing, Counseling and Treatment Access to clients at high risk for HIV or other STI infection with an emphasis on risk reduction. Some of the essential duties include: Provides HIV pre and post test counseling or syphilis screening, including risk assessments for drug abuse, sexually transmitted diseases (STDs) and mental health. Links clients to care and other support services as needed, including those at-risk individuals who test negative for HIV.
Makes referrals to the appropriate services and service providers as needed. Inputs client information using specific software as required (e.g. Casewatch). Supports billing activities as required. Processes all progress notes, client admissions and discharges. Maintains comprehensive, complete and accurate chart documentation. Drives the mobile unit to outreach sites as required and ensure its proper maintenance.
QUALIFICATIONS:
High school diploma is required. College education in the health or social services field preferred. Two years of related experience are required. HIV 104, HIV 500, HIV 501, Orasure and CPR trainings are required. Phlebotomy, proper driver license for van operation. Bilingual (English-Spanish/ English-Creole) is preferred. Computer knowledge should include Microsoft Word and Excel. Excellent communication, organizational and team work skills. Ability to work with multicultural and diverse population is required. Own transportation required. Driving of a 27 ft. mobile unit required.
SALARY: $11.00-15.00 Per Hour

| CASE MANAGEMENT SERVICES SUPERVISOR |
DESCRIPTION:
The Case Management Services Supervisor is responsible for the overall day-to-day administration of programs including Medical Case Management, PAC Waiver Case Management, and other assigned programs. Some essential duties include: Coordinates the hiring, training, and supervision of assigned staff. Coordinates service delivery with other departments as necessary. Maintains relationships with community groups and organizations at the local and state levels to ensure coordinated service delivery. Establishes as appropriate and ensures compliance with all standards of care, clinical and administrative procedures. Performs Quality Assurance and record reviews. Develops and coordinates the identification, organization and assessment of resources. Responsible for coordinating site visits, monitoring visits, and audits from funders. Coordinates reviews and monitors all billings, budgets and contracts. Ensures all documentation and billing is accurate, timely, legible and complete. Ensures productivity is equitably, reasonably and regularly produced by each person within the areas assigned. Maintains an active caseload as defined by management. Responsible for the control and reporting of monthly bus passes. Participates in grant writing and contracts management for the programs. Develops and maintains relevant policies and procedures. Facilitates staff training sessions as required.
QUALIFICATIONS:
Bachelor’s Degree in Social Work or related field required, Master’s Degree in Social Work or related field preferred. At least four years of case management experience with HIV/AIDS service provider organizations is preferred. Five years staff coordination experience is required. Case Management Medicaid Provider Certification is required. Completed application and fingerprinting submitted to supervisor within 30 days of hire and providership obtained within 90 days of hire if not already Medicaid PAC Waiver Provider. Bilingual (English-Spanish/ English-Creole) is highly recommended. Computer knowledge should include Microsoft Word, Excel and Patient Care Systems. Good organizational and team work skills. Excellent communication, decision making and problem solving skills are needed. Ability to work with multicultural and diverse population is required.
SALARY: $35,000- $46,000
