National Case Management Week is the second full week of October beginning on Sunday. Through National Case Management Week, Care Resource hopes to educate the community about the tremendous value our case managers bring to the successful delivery of health care.

Case management professionals play an increasingly significant role in the health care industry. As expert advisors and advocates, they shepherd our patients through the often intimidating maze that is modern health care, finding the optimum value for the patient and the reimbursement source while helping physicians through the identification of resources and services.

With the Affordable Care Act (ACA), Care Resource’s case management team also helps align those in the community with the right program. Case managers play an enhanced role to those navigating the healthcare system potentially for the first time.

Moreover, case managers are uniquely positioned to make that strong connection between helping patients navigate insurance choices, as well as assisting them in understanding whatever circumstance or condition brought them to need those services in the first place. They also can help make patients better consumers to use the tools that are available. Case managers are an essential link and a real advocate for members in that space.

Case Management at Care Resource is a client-centered collaborative process that works with patients to meet their health and care goals. Through assessing, planning, implementing, coordinating, monitoring and evaluating available options and services, case managers help assure one receives excellent care from available resources. Anyone may ask for Case Management at Care Resource.

According to Rafael Jimenez, Director of Case Management Services at Care Resource, “Our case managers encourage self-care and adherence to one’s medication regimen. Through support and emphasis on client directed goal setting, our case managers empower our patients to remain as independent as possible.  They refer those in the community to services including primary medical care, prescription drug assistance programs, housing assistance, food programs and many others.

Care Resource is a Federally Qualified Health Center (FQHC) that provides comprehensive primary medical and preventive care, including oral and mental health/substance abuse services to all individuals in our community.
