A COMMITMENT IN PROVIDING AN EXCEPTIONAL PATIENT PORTAL SERVICE EXPERIENCE

ausline_teamCare Resource’s Patient Portal provides a quality experience to our patients/clients health care needs.  This portal allows one to connect with Care Resource in a convenient, safe, and secure environment to communicate, access medical records, schedule an appointment, renew medications and more! Please take a minute to become familiar with these commonly asked questions about the Patient Portal. The answers can help make one’s experience navigating the portal more effective.

ARE YOU HAVING A PROBLEM CONNECTING TO THE PATIENT PORTAL?

Please send an e-mail to patientportal@careresource.org stating your service needs.  A reply will be given within 24 – 48 hours.

*Do not send personal information such as medical conditions or concerns, medical or insurance account information, financial information or social security number information to this general inquiry e-mail address. 

DO YOU NEED YOUR PASSWORD RESET OR HAS YOUR PIN HAS EXPIRED?

A PIN # is only good for 24 hours from the day of issue.  If the 24 hours has expired, a new PIN # must be reissued.  This cannot be done over the phone. One must come into a Care Resource health center to obtain the new PIN #. 

DO YOU NEED HEALTH INFORMATION, A MEDICAL REQUESTS, PRESCRIPTION REFILL INFORMATION OR TO MAKE AN APPOINTMENT REQUEST?

Please send a secure email message through the Patient Portal to your provider of choice or appropriate Patient Portal page.  A Care Resource Medical provider will reply within 24-48hrs.

EMERGENCY QUESTIONS OR EMERGENCY REQUESTS

The patient portal is not to be used for any form of inquiry referencing an emergency.  Please do not leave messages of an urgent matter or emergency on the portal. If you have an EMERGENCY, please contact 911 immediately or visit the nearest hospital emergency room.
