The Nutritionist is responsible for providing education and counseling to clients concerning nutrition related issues to improve health and overall well-being.
Understands and implements the philosophy, objectives, policies, and practices of the Agency
Inputs patient’s information using specific software as required (i.e. Intergy, Casewatch, Provide)
Documents all patient interactions in patient’s electronic health record (EHR)
Ensures accurate financial data are captured in agency’s database (i.e. Intergy, Provide) as required
Prepares necessary program reports and records as requested
Reviews medical provider schedules to identify patients who may benefit from nutrition services and notifies medical providers to request referrals as necessary
Reminds support staff, case managers, and providers on a regular basis of nutrition services and encourage issuing of referrals as appropriate
Assesses nutritional status of clients
Assists clients in developing a nutritional plan that addresses pertinent nutritional needs
Ensures follow-up assessments are accomplished in accordance with grantor requirements
Provides nutritional consultation when there are any changes that warrant further nutritional interventions
Documents patient’s financial, physical and/or environmental constraints as it relates to nutrition and access to food in efforts to achieve nutritional goalsProvides necessary referrals for other services and service providers as appropriate (i.e. food bank, pharmacy, case management and medical care)
Provides educational counseling to patients (i.e. exercise, tobacco cessation, and alcohol/drug abuse) when appropriate
Educates clients utilizing evidenced-based medical nutrition therapy and principles
Maintains comprehensive, complete and accurate chart documentation in accordance with agency and funder’s requirements
Coordinates and provides care with appropriate feedback to allied health care professionals when warranted (MDs, ARNP’s, PA’s, RNs, Case- Managers and Pharmacy staff)
Keeps current in trends and developments in the field of nutrition through continuing education
Participates in agency Quality Assurance programs
Participates in staff training sessions as required by the agency and the funding source(s)
Participates in agency developmental activities as requested
Other duties as assigned.
Ensures proper hand washing according to Centers for Disease Control and Prevention guidelines
Understands and appropriately acts upon assigned role in Emergency Code System, if assigned
Understands and performs assigned role in agency’s Continuity of Operations Plan (COOP).
Bachelor’s Degree in Nutrition/Dietetics. Three years of related experience preferred. A minimum of 2 years nutrition experience in a community health center highly desired. Continuing education for licensure requirements. Florida State License required. Registered Dietitian is required (RD). Bilingual (English-Spanish or English-Creole) is preferred. Computer knowledge should include Microsoft Word and Excel. Excellent communication, organizational and decision-making skills. Good problem solving and teamwork skills. Ability to work with multicultural and diverse population is required. The responsibility for external contacts is frequent and important. Own transportation is required.
